Abstract
. BACKGROUND AND OBJECTIVE: Mitomycin-C (MMC) has been shown to improve the surgical success of trabeculectomy; however, the advantages of MMC have been evaluated almost entirely as an adjunct to limbal-based trabeculectomy. This study evaluated the efficacy and safety of fornix-based trabeculectomy with MMC for glaucomatous patients. . PATIENTS AND METHODS: Between January 1993 and April 1995, 71 patients underwent fornix-based trabeculectomy with topical application of 0.4 mg/ml of MMC for 3 minutes. The conjunctiva-Tenon's capsule flap was spread over the limbus and sutured in order to create a visible crease with a watertight closure. The mean follow-up time was 14.5 months. . RESULTS: The mean intraocular pressure (IOP) before surgery was 32.4 ± 9.7 mm Hg. The average postoperative IOP was 14.04 ± 9.57 mm Hg. An IOP of 20 mm Hg or less was observed in 57 eyes (80%). Postoperatively, 37 eyes (52%) required no additional medical therapy. One month after surgery, only 2 patients had wound leakage with hypotony and choroidal detachment. Two eyes (3%) had suprachoroidal hemorrhage with loss of vision. A conjunctival buttonhole occurred in 2 eyes (3%), but only 1 persisted more than a month. . CONCLUSIONS: Fornix-based trabeculectomy using intraoperative application of 0.4 mg/ml of MMC for 3 minutes was found to be as safe and effective as limbal-based trabeculectomy with MMC.

PURPOSE: To examine the medium-term outcomes of fornix-based trabeculectomy (TLE) performed by a single surgeon and to analyze outcome in 3 glaucoma subtypes. METHODS: Consecutive prospective case series study to examine the 6-month outcome of 106 eyes [primary open-angle glaucoma (OAG), exfoliation glaucoma (XFG), and inflammatory glaucoma (ING)] that underwent TLE with mitomycin C performed by a single surgeon from September 2009 to September 2010 at Tokyo University Hospital. Main outcome mea...

OBJECTIVE: The objective of the study was to compare the efficacy and safety of topical dexamethasone sodium phosphate 0.1% (Sterodex) to diclofenac sodium 0.1% (Voltaren Ophthalmic) after combined phacotrabeculectomy with mitomycin C. METHODS: This prospective, randomized, double-masked clinical trial includes 42 patients with glaucoma and cataract who underwent a combined phacotrabeculectomy. Patients were randomly assigned to postoperative treatment with topical diclofenac sodium 0.1% or topi...

Human amniotic membrane (AM) is the innermost layer of the placental membrane. While the tough, semitransparent nature of AM is valuable as a graft for ocular surface reconstruction, it is the unique inherent biological action of AM during embryogenesis that has spawned increasing interests over the past decade in ophthalmology. The biological actions known to AM include antiinflammatory, antiscarring, anti-angiogenic, and growth-promoting effects. Several investigators have explored the clinica...

Trabeculectomy with an antimetabolite is considered the “gold standard” for the surgical management of glaucoma. Surgical management for glaucoma was first described in 1857 by Von Graefe, who reported that by removing a large piece of the iris he could help many patients with glaucoma. In 1909, Elliot described a full-thickness filtering procedure by using a trephine to make an anterior sclerectomy under a conjunctival flap, coupled with a peripheral iridectomy. Uncontrolled transclerostomy flo...

Purpose To determine the incidence of and risk factors for hemorrhagic complications in patients on anticoagulation (ACT) or antiplatelet therapy (APT) having glaucoma surgery. Design Retrospective case-control study. Methods Medical records of patients who had glaucoma surgery between July 1, 1998 and March 31, 2005 were reviewed. Patients who either used ACT/APT continuously throughout the perioperative period or discontinued its use prior to surgery were compared to case-matched control patie...

BACKGROUND AND OBJECTIVE: To compare the safety and efficacy of fornix-based trabeculectomy with corneal valve to traditional limbal-based trabeculectomy with mitomycin C (MMC) in eyes with open-angle glaucoma. PATIENTS AND METHODS: In this retrospective, nonrandomized, comparative study, 42 eyes that underwent limbal-based trabeculectomy with MMC were compared with 32 eyes with fornix-based trabeculectomy with corneal valve with MMC. Success was defined as final intraocular pressure (IOP) of at...

PURPOSE: To compare trabeculectomy with mitomycin C (T+MMC) and Ahmed glaucoma implant with MMC (AGI+MMC) in treatment of aphakic glaucoma in children below 16 years of age. DESIGN: Randomized clinical trial. PARTICIPANTS: Subjects less than 16 years of age who had previously undergone anterior lensectomy and vitrectomy for treatment of congenital cataract with unresponsive aphakic glaucoma (at least on 2 medications) were allocated in 2 groups: trabeculectomy with MMC (group A, 15 eyes) and AGI...

PURPOSE: To describe the use of amniotic membrane (AM) transplantation for the repair of conjunctival buttonholes during trabeculectomy with mitomycin C. METHODS: Four eyes of 3 patients with thin conjunctiva, precluding watertight conjunctival closure at the incision site with suturing, underwent intraoperative AM grafts over the leaking areas. RESULTS: In all eyes, a functional, nonleaking bleb was achieved. At the latest follow-up (8 to 30 mo), all eyes had intraocular pressures of 12 mm Hg o...

Purpose To evaluate the safety and efficacy of trabeculectomies performed by ophthalmology residents at a metropolitan county hospital, under the supervision of attending physicians. Methods A retrospective analysis of resident-performed trabeculectomies at the San Francisco General Hospital from the period of 1994 to 2004 was performed. The preoperative and postoperative ocular data of 50 eyes in 35 patients were evaluated. Of the 50 cases, 47 procedures were performed as primary trabeculectomi...

. Purpose: To compare the intermediate-term efficacy of 5-fluorouracil (5-FU) and Mitomycin C (MMC) as adjunctive antimetabolites in neovascular glaucoma (NVG) filtration surgery. Methods: Forty consecutive eyes of 40 patients with NVG refractory to medical therapy were randomized to receive antimetabolite-augmented trabeculectomy. Eighteen eyes received postoperative 5-FU (5-FU group) and 22 eyes received intraoperative, low-dose (0.2 mg/ml) MMC for 2 mins (MMC group). The main outcome measure ...
