A 69-year-old man had hospitalized for acute exacerbation of chronic heart failure (CHF). He had suffered from CHF, chronic kidney disease and chronic obstructive pulmonary disease (COPD) for 10 years. On admission, his transthoracic echocardiograms showed diffuse hypokinesis with low ejection fraction (EF), which declined from 46% to 17% in 6 months. The etiology of cardiac dysfunction was unknown until now. In his clinical history, his EF was relatively preserved (46%) around half a year before.
