Eleven autogenous saphenous vein loops have been used in 9 patients on haemodialysis in order to gain access to the circulation. All the patients lacked peripheral vessels available for alternative procedures. These grafts have functioned satisfactorily for over 18 months and have proved technically easy to construct and efficient to use. Bilateral nephrectomy was perjormed in 53 patients on regular haemodialysis. The indications were pyelonephritis in 30, polycystic kidneys in 6, glomeruloneph-ritis in 7, uncontvollable hypertension in 9 and horseshoe kidney in 1. In 87 per cent of cases the operation was carried out as a separate procedure prior to transplantation. Abstract The rate of loss of albumin to the tissue spaces (measured as transcapillary escape rate) rose by more than 300% in patients with septic shock, and the average increase within 7 h of cardiac surgery was 100%. Ultrasound examinations of the pelvis and kidney have been carried out following 68 renal transplants. In 16 patients moderate sized or large perirenal or pelvic fluid collections were demonstrated. The composition of the fluid was lymph in seven cases, pus in four, blood in three and urine in two.
