Within pregnancies occurring between 1986-2017 in Dutch kidney transplant recipients (KTR), we retrospectively compared short-term maternal and fetal outcomes between patients on calcineurin inhibitor (CNI) based (CNI+) and CNI free -immunosuppression (CNI-). We identified 129 CNI+ and 125 CNI- pregnancies in 177 KTR. Demographics differed with CNI+ having higher body mass index (p=0.045), shorter transplant-pregnancy interval (p<0.01), later year of transplantation and -pregnancy (p<0.01). Seru...

Allogeneic islet transplantation in type 1 diabetes requires lifelong immunosuppression to prevent graft rejection. This medication can cause adverse effects and increases the susceptibility for infections and malignancies. Adoptive therapies with regulatory T cells (Tregs) have shown promise in reducing the need for immunosuppression in human transplantation settings but have previously not been evaluated in islet transplantation. In this study, five patients with type 1 diabetes undergoing int...

Evidence suggests that liver graft quality impacts on post-transplant recurrence of hepatocellular carcinoma (HCC). As of today, selection criteria only use variables related to tumor characteristics. Within the Scientific Registry of Transplant Recipients, we identified patients with HCC who underwent liver transplantation between 2004 and 2016 (development cohort, n=10,887). Based on tumor recurrence rates, we fitted a competing-risk regression incorporating tumor- and donor-related factors, a...

We read with interest the study of Van den Eynde et al. on the effect of perfusion solutions on liver transplant outcome.(1) Graft dysfunction was analyzed applying two indicators: Early Allograft Dysfunction (EAD, a dichotomous system based on transaminase, bilirubin, and coagulation cut-off values)(2) and Model of Early Allograft Function (MEAF, a continuous score based on the same variables as EAD).( 3).

Prediction of hepatocellular carcinoma (HCC) recurrence after liver transplantation (LT) with knowledge of explant data is important for guiding post-LT surveillance and treatment. The RETREAT score was recently introduced for this purpose, but has not been validated outside the USA. In a retrospective single-center study of 169 consecutive patients undergoing LT in Gothenburg, through 2000-2017 (mean age 57 years, 80% men), there were 34 HCC recurrences during a median 4.6-year follow-up. The 5...

Background null Full-left-full-right split liver transplantation (FSLT) for adult recipients, may increase availability of liver grafts, reduce waitlist time, and benefit recipients with below-average body weight. However, FSLT may lead to impaired graft and patient survival. This study aims to assess outcomes after FSLT. null Methods null Five databases were searched to identify studies concerning FSLT. Incidences of complications, graft- and patient survival were assessed. Discrete data were p...

BACKGROUND AND AIMS Pre-liver transplant (LT) chronic kidney disease (CKD) has emerged as a leading cause of postoperative morbidity. We aimed to report the prevalence, associated risk factors and clinical outcomes in patients with pre-LT CKD. METHODS Meta-analysis and systematic review were conducted for included cohort and cross-sectional studies. Studies comparing healthy and CKD patients pre-LT were included. Outcomes were assessed with pooled hazard ratios. RESULTS 15 studies were included,...

Intra-operative blood salvage (IBS) reduces the use of allogeneic blood transfusion. However, safety of IBS during liver transplantation (LT) for hepatocellular carcinoma (HCC) is questioned due to fear for dissemination of circulating malignant cells. This study aims to assess safety of IBS. HCC patients who underwent LT from January 2006 through December 2019 were included. Patients in whom IBS was used were propensity score matched (1:1) to control patients. Disease-free survival and time to ...

The reported incidence of biliary complications after living donor liver transplantation (LDLT) is 11%-35% [1, 2] which are twice as common with deceased donor LT (DDLT) [1].The greater incidence of biliary complications further contribute largely to the higher post-operative morbidity, prolonged hospital stay and increased re-hospitalization rates in LDLT patients [3].

Definitions for chronic lung allograft dysfunction (CLAD) phenotypes were recently revised (2019 ISHLT consensus). Post-CLAD onset phenotype transition may occur as a result of change in obstruction, restriction, or RAS-like opacities (RLO). We aimed to assess the prevalence and prognostic implications of these transitions. This was a single-center, retrospective cohort study of bilateral lung transplants performed in 2009-2015. CLAD phenotypes were determined per ISHLT guidelines. CLAD phenotyp...
