Pulmonary embolism and infarction with a paradoxical thrombus visualised in both atria

A 59-year-old woman presented with a sudden onset of breathlessness and chest pain. An echocardiography and CT scan showed pulmonary embolism and infarction with a paradoxical thrombus visualised in both atria.
