A case of cerebral amyloid angiopathy and posterior reversible encephalopathy syndrome combined with chronic kidney disease
Abstract
症例は68歳, 男性. X-6年にIgA腎症と診断されたが, 通院を自己中断していた. X年, 頭痛・失読が出現し, 当院に搬送された. 受診時血圧245/112mmHg, 血清Cr 8.60mg/dLと著明高血圧と末期腎不全を呈していたが, 神経所見は消失していた. 頭部MRIで多発性微小出血後変化, 白質血管性浮腫が認められ, 脳アミロイドアンギオパチー (cerebral amyloid angiopathy: CAA) に可逆性後頭葉白質脳症症候群 (PRES) が合併したと診断した. 白質病変の鑑別診断として, 炎症性CAAも想定された. 著明高血圧に対し降圧療法を開始し, 末期腎不全に対し血液透析を導入した. 次第に血圧は安定し, 第14病日の頭部MRIにおいて, 白質の血管性浮腫部は改善傾向を示した.
