Established and emerging applications of PET-CT in infection and inflammation

1044 Learning Objectives 1. To review the role of FDG PET-CT in suspected infection and inflammation. 2. To illustrate the range of conditions in which PET-CT can add value in patient management. FDG PET-CT can be used to investigate possible inflammation or infection in a variety of clinical situations. The technique often has a complementary role when there is clinical uncertainty or equivocal findings on other imaging modalities. Established applications include the assessment of patients with pyrexia of unknown origin, differentiating cerebral infection (toxoplasmosis) from lymphoma in immuno-compromised patients and evaluating suspected vascular graft infection. PET-CT also has an increasing role in imaging of non-infectious inflammatory diseases such as vasculitis, sarcoidosis and inflammatory bowel disease where it can help define the extent of disease and monitor treatment response. Emerging applications include the use of FDG PET-CT in the diagnostic work-up of high-risk patients with gram-positive bacteraemia, assessment of suspected prosthetic joint infection and evaluation of the extent and location of infection in patients with acquired immuno-deficiency syndrome (AIDS). Novel tracers such as 68Gallium-Citrate and FDG labelled white blood cells offer the promise of highly accurate imaging in the future. This exhibit will illustrate the established and emerging applications of PET-CT in infection and inflammation and review the utility of the technique in a variety of different clinical scenarios. Conclusion: FDG PET-CT can have a complementary role in the investigation of selected patients with suspected infection or inflammation. PET-CT using novel tracers may become the radionuclide imaging procedure of choice in the future
