Background: Polymyalgia rheumatica (PMR) is a common inflammatory condition of elderly persons. Clinical symptoms respond to low-dose glucocorticoids (GC), but treatment is often required for several years. 2015 EULAR/ACR recommendations1) recommend considering early introduction of methotrexate (MTX) in addition to GC, particularly in patients at a high risk for relapse and/or prolonged therapy. However, risk factors for prolonged therapy are not clear yet. Objectives: We investigated predictiv...

Background A recent meta-analysis shows patients with rheumatoid arthritis (RA) are at increased risk of lung and lymphoid malignancies compared with general populations1). Rheumatologists perform cancer screening before treatment of biological disease-modifying anti-rheumatic drugs (DMARDs), and do routine blood, urine and imaging tests to find adverse effects of DMARDs. However, they can’t always find malignant solid tumors (cancers) at the early stage in patients with RA. Objectives We examin...

Background The gastrointestinal tract (GIT) is the second most common internal organ affected by systemic sclerosis (SSc). The rate of SSc patients who develop severe GIT symptoms is lower than 10%, although various degrees of chronic intestinal pseudo-obstruction (CIPO) may occur in as many as 40% of cases (1.2). Objectives To report an autopsy case of SSc with severe intestinal involvement and review the associated literature. Methods We will present the clinical features and autopsy findings ...

Background As disease modified anti-rheumatic-drugs (DMARDs) were the mainstay in the treatment of rheumatoid arthritis (RA), the opportunity for general physicians to also prescribe DMARDs has recently increased in Japan. After DMARDs were initiated by specialists, subsequent drug prescription by general physicians during remission should be expected 1 . However, general physicians are more likely than rheumatologists to feel it difficult to accept patients taking even conventional synthetics (...

Background As Japan is a super-aged society, we have many chances to care for elderly patients in our hospital. Elderly-onset rheumatoid arthritis (RA) (onset age >60 years) may present similar symptoms to those of polymyalgia rheumatica (PMR). We consider that differential diagnosis of RA and PMR is more difficult in patients over 75 than those under 74 in clinical practice. Anti-cyclic citrullinated peptide antibody (ACPA) was reported to be a helpful tool in the differential diagnosis of EORA...

Background Polymyalgia rheumatica (PMR) is a common inflammatory condition of elderly persons. Clinical symptoms are improved by low-dose glucocorticoids (GCs). However, most patients require long-term GC therapy, because relapses are common. The recent recommendations for the management of PMR 1) suggested early introduction of methotrexate (MTX) in addition to GCs in patients with a relapse, and/or without a significant response to GC, or experiencing GC-related adverse events. Objectives The ...

Rheumatoid arthritis (RA) is a complex chronic illness requiring continued medical care. During the past decade, the therapeutic options for RA have increased significantly; these often have a higher risk of adverse effects and are more expensive than traditional drugs. Rheumatologists may hence face difficulties when deciding on the optimal modality in initiating or changing treatment. The aim of this study was to explore the Japanese physicians' usual style of and preferences for decision maki...

Abstract We report on an outbreak of Group A Streptococcus (GAS) pharyngitis among university students in a judo club. Eventually, 14 of total 23 club members developed acute pharyngitis clinically. In a span of 15 days in April 2013, 12 students visited our hospital complaining of sore throat and high fever. All were men with a median age of 19.5 years (interquartile range, 18–21). The rapid streptococcal antigen test was positive in 3 of 4 patients (75%) without previous antibiotic treatment, ...
