The diagnosis of autoimmune pancreatitis (AIP) is often challenging and distinguishing it from chronic pancreatitis or pancreatic cancer (PC) could be very difficult. In particular, AIP can present at CT scan, MRI or endoscopic ultrasound (EUS) as a focal mass, sometimes with no other specific peculiarities. Furthermore, current international guidelines1 2 consider EUS only as a way to obtain cyto-histological specimens to exclude malignancy, regardless to its diagnostic capacity based on morpho...

Importance Branch-duct intraductal papillary mucinous neoplasms (BD-IPMNs) are common pancreatic preneoplastic lesions, but their surveillance is not personalized. Objective To investigate patient- and cyst-related factors associated with progression into worrisome features (WFs) or high-risk stigmata (HRS) categories of BD-IPMNs. Design, setting, and participants Cyst- and patient-related factors of consecutive BD-IPMNs without WFs or HRS in 540 patients diagnosed from 2009 to 2018 with at leas...

The role of fibre intake in the management of patients with pancreatic disease is still controversial. In acute pancreatitis, a prebiotic enriched diet is associated with low rates of pancreatic necrosis infection, hospital stay, systemic inflammatory response syndrome and multiorgan failure. This protective effect seems to be connected with the ability of fibre to stabilise the disturbed intestinal barrier homeostasis and to reduce the infection rate. On the other hand, in patients with exocrin...
