Minimally invasive surgery (MIS) has many benefits, in the form of reduced postoperative morbidity, improved recovery and reduced inpatient stay. It is imperative, however, when new techniques are adopted, in the context of treating oncology patients, that the oncological efficacy and safety are established rigorously rather than assumed based on first principles. Here we have attempted to provide a comprehensive review of all the contentious and topical themes surrounding the use of MIS in the ...

Introduction/Background MIRRORS is a UK based prospective feasibility study opened June 2020, following ethics approval. Its purpose is to establish the feasibility of launching a randomised control trial (RCT) of Robotic interval debulking surgery for ovarian cancer (including cancer of the fallopian tube & peritoneum) MIRRORS-RCT in the future. MIRRORS will focus on the feasibility of obtaining consent from women and the acceptability of Robotic interval debulking surgery for advanced ovarian ...

Introduction/Background The European Society of Gynaecological Oncology(ESGO) and The British Gynaecological Cancer Society(BGCS) recommend laparoscopic surgery for treatment of endometrial cancer. Conversion rates to open can be high when performing surgical staging, particularly in patients with high BMI. The Royal Surrey in Guildford has been performing laparoscopic surgery for endometrial cancer since 2002 and introduced robotic assistance in January 2010. Since then >1400 gynaecological onc...

We identified several CH–π donor–acceptor pairs involving amino acid side chains with less polarized C–H bonds at a solvent-exposed site between the strands of a β-hairpin peptide. Therein, we observe a distance-dependent induction of CH–π interaction within the aliphatic-aromatic amino acid pair. Our results also suggest an interplay of hydrophobicity and CH–π interaction in dictating the stability of β-hairpins, where a leucine-tryptophan pair contributes −1.14 kcal mol−1 to the overall folded...

Abundant n→π* interactions between adjacent backbone carbonyl groups, identified by statistical analysis of protein structures is predicted to play an important role in dictating the structure of proteins. However, experimentally testing the prediction in proteins has been challenging due to the weak nature of this interaction. By amplifying the strength of n→π* interaction via amino acid substitution and thioamide incorporation at a solvent exposed β-turn within the proteins GB1 and Pin 1 WW do...

Hypotension following major abdominal surgery is common, and once hypovolaemia has been optimally treated, is often due to vasodilation which can be treated with vasopressor infusions. There is unpredictability in the dose and duration of post-operative vasopressor infusions, and factors associated with this have not been determined. We present a case series of consecutive patients who received major gynae-oncology surgery delivered within an Enhanced Recovery After Surgery (ERAS) pathway at a s...

On March 11, 2020 the COVID-19 outbreak was declared a 'pandemic' by the World Health Organization COVID-19 is associated with higher surgical morbidity and mortality An array of guidelines on the management of cancer during this pandemic have been published since the first reports of the outbreak This narrative review brings all the relevant information from the guidelines together into one document, to support patient care We present a detailed review of published guidelines, statements, comme...

BACKGROUND: We present the largest UK single institute robotic radical hysterectomy (RRH) case series for the management of cervical cancer (CC). METHODS: Data were collected on women who had a RRH as primary treatment for stage 1b1 CC between December 2009-December 2018. RESULTS: 90 women had a robotic hysterectomy. Five-year follow up data was available for 30%. The disease-free survival at five years was 89.6%. Overall survival at 3-years and 5-years for death from any cause was 96.1% and 91....

The unique physicochemical properties of a thioamide bond, which is an ideal isostere of an amide bond, have not been fully exploited because of the tedious synthesis of thionated amino acid building blocks. Here, we report a purification-free and highly efficient synthesis of thiobenzotriazolides of Fmoc-protected and orthogonally protected 20 naturally occurring amino acids including asparagine, glutamine, and histidine. The near-quantitative conversion to the respective thioamidated peptides ...

Abstract Checkpoint immunotherapy has revolutionised the way that melanoma is treated and has also shown significant effectiveness in lung, bladder, renal, and head and neck cancers. At the present time, trials of checkpoint immunotherapy in cervical cancer are at early phases, but there is very good rationale for pursuing this as a treatment option, especially as cervical cancer is a virally driven cancer and therefore should be recognised by the immune system as being foreign. This review expl...
