An 80-year-old man was admitted due to biliary stricture with autoimmune pancreatitis. Although radiographical examinations suggested Immunoglobulin G4-related sclerosing cholangitis (IgG4-SC), punched biopsies from the bile duct revealed adenocarcinoma. IgG4-related cholecystitis is a type of IgG4-related disease in the gallbladder. Both diffuse and localized forms of the disease have been described. The former may present as diffuse thickness of the gallbladder wall in patients evaluated for autoimmune pancreatitis or IgG4-related sclerosing cholangitis. Localized IgG4-related cholecystitis is difficult to diagnose; it must be distinguished from gallbladder cancer, which is often challenging.
