Pancreatic ductal adenocarcinoma (PDAC) is one of the deadliest cancers, with an overall five-year survival rate of 11%. Potential curative resection is possible if the tumor is detected at an early stage, with a five-year survival rate of 42%. The only current FDA-cleared biomarker for PDAC is the carbohydrate antigen 19-9 (CA19-9), which is intended for monitoring response to therapy but not for early detection. CA19-9 blood tests have varying sensitivity to detect PDAC and are prone to false ...

PURPOSE/OBJECTIVE(S) Loss of the tumor suppressor gene SMAD4 is a critical genetic alteration in pancreatic carcinoma (PC). We hypothesized that SMAD4 status in PC is associated with outcomes in patients who received neoadjuvant (NARx) or adjuvant (ARx) radiotherapy. MATERIALS/METHODS PC patients who underwent surgical resection at two high-volume centers following NARx-or those treated with ARx-between 2008-2019 were identified. SMAD4 status was determined based on immunohistochemical staining ...

Background null A paucity of research regarding the psychosocial outcomes after TPIAT exists. null Methods null Adults (>18 years), adolescents (13-18 years), and children (5-12 years) with their parents were administered questionnaires at the time of evaluation for TPIAT and 1-year postsurgery to assess psychosocial outcomes. null Results null A total of 13 adults (6 male, 46%; mean age 35.2 years) and 9 children/adolescents (4 female, 44.4%; mean age 11.78 years) with CP were included in the s...

Introduction null Preoperative autophagy inhibition with hydroxychloroquine (HCQ) in combination with gemcitabine in pancreatic adenocarcinoma (PDAC) has been shown to be safe and effective in inducing a serum biomarker response and increase resection rates in a previous phase I/II clinical trial. We aimed to analyze the long-term outcomes of preoperative HCQ with gemcitabine for this cohort. null Methods null A review of patients enrolled between July 2010 and February 2013 in the completed pha...

AIM To assess the predictors and influence of resection margins and the role of neoadjuvant and adjuvant therapy on survival for a national cohort of patients with resected pancreatic cancer. METHODS Using the National Cancer Data Base between 2004 and 2016, 56,532 patients were identified who underwent surgical resection for pancreatic adenocarcinoma. Univariate and multivariate models were employed to identify factors predicting R0/R1 resection and assess the impact on survival. RESULTS In tot...

Abstract null null Background null Intraperitoneal drain placement decreases morbidity and mortality in patients who develop a clinically relevant postoperative pancreatic fistula (CR-POPF) following pancreaticoduodenectomy (PD). It is unknown whether multiple drains mitigate CR-POPF better than a single drain. We hypothesize that multiple drains decrease the complication burden more than a single drain in cases at greater risk for CR-POPF. null null null Methods null The Fistula Risk Score (FRS...
