Introduction null AIDS Drug Assistance Programs (ADAPs) provide financial support for medical care for people living with HIV (PLWH) in the US. Federal policy requires that clients recertify for the program every 6 months, which has been described as a barrier to care access. Our objective was to describe the prevalence and factors associated with ADAP disenrollment in Washington State. null Methods null Between 2017 and 2019, we categorized ADAP clients by the success of their recertification a...

Background: Assisted partner services (APS) are a longstanding component of public health efforts to control HIV in the United States, but their contemporary effectiveness is uncertain. Methods: The study surveyed U.S. state and local health departments that reported > 300 new HIV diagnoses in 2018 and were participating in phase 1 of the Ending the Epidemic Initiative. The study collected data on the content and organization of APS and aggregate data on APS outcomes for 2019. Analyses defined t...

Mycoplasma genitalium is a sexually-transmitted bacterium associated with non-gonococcal urethritis (NGU) in men and cervicitis, endometritis, and pelvic inflammatory disease in women. Effective treatment is challenging due to the inherent, and increasingly acquired, antibiotic resistance in this pathogen. In our treatment trial conducted from 2007 - 2011 in Seattle, WA, we demonstrated poor efficacy of azithromycin (AZM) and doxycycline (DOX) against M. genitalium among men with NGU. In the cur...

BACKGROUND Pharyngeal gonorrhea is relatively common. However, the duration of untreated pharyngeal gonorrhea is unknown. METHODS From March 2016 to December 2018, we enrolled 140 MSM in a 48-week cohort study. Participants self-collected pharyngeal specimens and completed a survey weekly. Specimens were tested using a nucleic acid amplification test at the conclusion of the study. We estimated the incidence and duration of infection. We defined incident infections as two consecutive positive te...

Background null The duration of rectal gonococcal and chlamydial infection remains unknown. This basic epidemiologic parameter is needed to understand transmission dynamics. null Methods null We conducted a prospective, longitudinal, observational, cohort study of 140 men who have sex with men (MSM) at-risk of gonorrhea and chlamydia acquisition. For 48 weeks, enrolled men collected rectal swabs (Aptima multitest kit) at home and responded to an electronic survey about sexual behavior and health...

BACKGROUND: In response to the COVID-19 pandemic, Washington State's Stay Home, Stay Healthy (SHSH) order was implemented on 3/24/2020. We hypothesized that pandemic mitigation measures might reduce sexually transmitted infection (STI) screening and/or transmission. METHODS: We used King County, WA STI surveillance and sexual health clinic (SHC) data from 1/1/2019-7/31/2020. We calculated mean weekly case counts for gonorrhea, primary and secondary (P&S) syphilis, male urethral gonorrhea, and ea...

Background null The time to RNA-based nucleic acid amplification tests (NAAT) clearance following treatment of NG at the pharynx remains undefined. null Methods null Between March 2019–2020, we enrolled 55 men who have sex with men (MSM) with untreated pharyngeal gonorrhea detected ≤14 days to self-collect daily pharyngeal swabs at home for 21 days after treatment. All men received ceftriaxone 250mg intramuscularly plus 1g azithromycin orally once at enrollment. We used Kaplan Meier estimates to...

Background null The prevalence of pharyngeal CT is low but its incidence and duration are unknown. A high incidence and/or duration may support the role of pharyngeal CT in sustaining CT transmission among MSM. null Methods null From March 2016 to December 2018 we enrolled MSM in a 48-week natural history cohort study in Seattle, Washington. Participants self-collected pharyngeal specimens weekly. We tested specimens using nucleic acid amplification testing (Aptima Combo-2) at the conclusion of ...
